I am requesting an appointment for a member of mine who only speaks Spanish and has limited reading skills. Member lives in buffalo, NY. I would like to attend the appointment with him. I have started filling in the application for him online. Re-entry number is 29946316.
Member was born in Puerto Rico. He has a birth certificate which is in 2 pieces-still legible. Not sure if he needs to send for a new one. I was told the replacement would be $40+ and member is on a very limited income.
