Sent form CMS 1763 a month ago. Checked with National Office-they never received. Called the Bellevue office and spoke with the female employee. She didn't know where the form was, & had no intention of finding it. Asked a lot of personal question, upon reflection, were none of her business. Private sector would have kicked her to the curb a long time ago. Turned the matter over to a lawyer.

Currently in this month May 2019, "customer have to get in line" very long line or comeback.

One worker is look tired or day dream, every time you come to this Bellevue office, one lady is working hard while another lady is look like a philosopher with no brain.

Let pay her more for better service.

Need a local number for making appointment when online problems are unresolved. A 2-hour wait on the phone is ridiculous.

The lack of a local phone number is grotesque and disgusting. I should not have to call a nation-wide number and wiat an hour and a quarter, to ask a 30 second question, that the national office can't answer, anyway.

Dear Ladies and Gentlemen, I visited your Office back in June 2017 and handed in the requested SSA-7182-OCR-SM Report to the US
SSA and one of your employees made a copy for me to sign and complete since the original was mailed to our German address and we did not return until 29.06.2017. My simple question is, is our account now in order and regulars payments will be forthcoming? Thank you for the expert and professional treatment we received during this visit in your office. The Employee was an Asian and very kind and helpful. Thank you! Please reply that all is in order.

I tried to call the local office to see if I needed to make an appointment to see an agent to dispute my medicare premium increase and to discuss applying for social security. There was no way to do that, nor any information about whether an appointment was necessary or benefitical
